This helpful tutorial has been written by drill. Thank you for that, enjoy.
Don't know if your request is still actual, i needed make some effected video also, so i can wrote here my :
Step by step tutorial how to batch :
All frames in Photoshop
-> First of all, you need split your screenshots and grayscale screenshots.
I made new folder in c:/ called ,,In Depth Of Field''. Open it and make another two folders in it. First can be called ,,Grayscale'', second ,,Final''
Then go copy/cut your all screenshots and paste them in C:\Depth Of Field, next is copy/cut your all grayscale screenshots and put them in C:\Depth Of Field\Grayscale
It should like be like this : picture
Note: Be sure your both screenshots (original, depth) have first same number.
In my screenshot they are : f70r100.0000000004.tga, f70r100.depth.0000000004.png. Otherwise your original and graysclae screens will be not synced.
-> Adding grayscale pictures into ImageReady
Open your ImageReady, go File->Import->Folder as Frames... In browser open folder where you have saved depth screenshots. In our case it is C:\Depth Of Field\Grayscale
ImageReady should open it without any problems. If it will ask for some skipping frames, don't allow it. We need have all depth-ed pictures. You can also check in animation window if you have all frames loaded there -> picture
-> Making layers from screenshots
Next step is making layers from grayscale screenshots. When we have frames in ImageReady, we need proceed it into Photoshop. Go File->Jump to->Adobe Photoshop. It should make that layers in photoshop. It should looks like this : picture. If you can see my first layer is called
f70r100.depth.0000000004.png. So 0000000004 is first layer and it is OK.
-> Load actions in Photoshop
Save my actually made set of actions (link) in your Photoshop Actions folder. In my case it is : C:\Program Files\Adobe\Photoshop 7.0 CE\Presets\Photoshop Actions.
Then in Photoshop go actions window, by arrow (button) click on ,,Load Actions...'' - picture. It should load actions in list. You can browse them in below of list -> picture
-> Batching
If we have loaded actions, we can proceed to batch all frames. Go File->Automate->Batch. In Batch window set all like is on screenshot and press OK.
It should start batching all frames step by step. When it is finished, it shows error that last layer cannot be deleted, click on stop, but it doesn't changing nothing cause it means for you all is finished.
Now you can go in ,,Final'' folder and check your Depth of Filed movie.

Hope i helped a bit, i know there is any possibilities to make effected video by other program, if you got hint you can write
